Normal onboarding standards will be applied unless you tell us differently in the ticket:
- We will NOT contact the employee prior to the Effective Date
- We will only contact the Offboarding Manager Contact you provide in this ticket until the Effective Date
- The Effective Date should be the date the person is expected to be on site to start work
- We will start to analyze requested asset and service assignments to determine all the tasks needed and see if any questions are raised (and contact the Onboarding Manager Contact if necessary)
- Please be sure to fill in appropriate existing employee names in the Building Access and System Access Model Name fields so we can use those existing employees as "models" for copying door access schedules and security group memberships.
- Be sure to specify in the description of the ticket any desired deviations from the above standards, including: where to forward e-mail (for the next 30 days), alternative arrangements for personal files, call forwarding (for next 30 days), etc.

Regular Employee
(Retirements, Terminations, Leaves, LTD etc.)

Normal offboarding standards will be applied unless you tell us differently in the ticket:
- We will NOT contact the employee, assuming the matter is private.
- We will only contact the Offboarding Manager Contact you provide in this ticket until the Effective Date.
- The Effective Date should be no later than the Last Day Worked. (It is *never* the last date on payroll, which can be much later.)
- We will start to analyze existing asset and service assignments to determine all the tasks needed and see if any questions are raised (and contact the Offboarding Manager Contact if necessary)
- User's home equipment (laptop, cellphone, aircard, etc.) must be returned to IT no later than 3 business days prior to the Effective Date.
- At the end of the Effective Date (or the next morning), the user's access to the network, building, email, voicemail, etc. will be disabled. The DUO MFA account will also be suspended to prevent application logon from alternate userid.
- After 30 days, the user's existing computing (AD), and email accounts, and all personal files (H or U drives) will be deleted
